Based on the invaluable feedback from my peers and instructors I have evolved my educational dilemma.
My online course should:
- Fill the knowledge gap of residents regarding the intrinsic CanMEDS roles
- Fill the skill gap by
- Provide opportunities for interaction between residents
- Provide opportunities to practice some of roles
- Provide Reflection opportunities
Proposed strategies:
- Required readings for each module: content of Royall College of physicians and surgeons website could be used as source
- Asynchronous discussions on scenarios common to surgical fields such as:
- Communication with patients and families in emergency surgery situations
- Explanation of surgical complications to the patients
- Shared decision making in surgery cases
- Handling conflict during operation with OR team
- Leading surgery team
- Group assignments: health advocating projects to practice health advocacy and collaboration skills.
- E-portfolio to reflect on the clinical encounters and the instructor can follow the progress of learners during the course.
- Final paper on one of one of the roles (the purpose of this task is to practice scholar role)

Hi Zahra. Glad to see you with a clear idea of you education dilemma. Your objective and solution of creating an online course for the orthopedic surgery residents to learn CanMEDs are clear and precise. I think you have covered most of the learning elements in your proposed online course: basic knowledge theories, required and optional readings, relevant scenarios and discussions, group assignment for collaboration and leadership.
Here are some of my suggestions:
1. Provide some literature review to support your problem statement. For example, how CanMEDs roles be important in orthopedic surgery residents, how previous literatures showed that they do face difficulties in learning CanMEDs and why online course is an appropriate choice.
2. For the group project evaluations, I think the group members can self-evaluated their own work by using SWOT analysis as self-assessment is an important ability as well. Here is an article related to SWOT analysis, hope it helps.

Hello Zahra,
The development of the soft skills is as important as other core medical knowledge and addressing the gaps is a good idea. Collaboration and leadership qualities are also a must have because patient care calls for interdisciplinary collaboration. Creating an online platform to get residents working together helps to promote the development of these qualities and they can easily transition it into their practice later on.
I will suggest that the scenarios or the case studies you will be creating should include a role for various experts, for instance, a patient admitted for femur fracture could have pre-existing comorbidities that needed to be stabilized before surgery can be done, this case will ensure that the surgeon call on other expert to consult and they all have to work together.
